Never Events are incidents that require full investigation under the Serious Incident framework. Failure to report a Never Event is unacceptable and can signal cultural and safety failings in an organisation.
Requiring medical treatment can be stressful enough on its own. However, suffering from a never event can have a huge emotional and physical impact on you or your loved ones, potentially requiring even further treatment and injury. The NHS defines never events as “serious incidents that are entirely preventable because guidance or safety recommendations providing strong systemic protective barriers are available at a national level and should have been implemented by all healthcare providers.”
A never event is medical negligence and arises from preventable mistakes being made by healthcare professionals. A few common examples of never events are as follows, but not limited to:
- Incorrect surgery performed
- Wrong site surgery is done
- Foreign objects left in the body after surgery
- Medication overdose
- Medication administered via the wrong route
- Misplacing nasogastric tubes
- Use of the incorrect prosthetics or implants
- Transfusion of incompatible blood types
- Organ transplant with incompatible organs
- Scalding with hot medical equipment
- Failure to provide a bed or shower with collapsible rails
If a never event occurs, you should be informed by a healthcare professional. However, if you feel that you or a loved one has been subject to a never event but were not informed, you are still able to make a claim.
